How to Register

We do our best to match you with a trainer as soon as possible. Most clients are matched with a trainer within a week or two after all paperwork has been submitted. However, please understand that many of our trainers are full time students with limited availability.

2. Please Turn in the following medical forms:

Note: Your PAR-Q and health history forms can be dropped off at Member Services in a secured labeled envelope (Attention Stephanie Belcher), faxed to 404.894.9982 (Attention Stephanie Belcher), or emailed to stephanie.belcher@crc.gatech.edu. 

4. Register and pay at the CRC Member Services Desk. 

  • All payments occur at the Member Services Desk.

Please Note: 

  • All steps must be completed before you will be able to meet with a trainer. 
  • A GIT FIT  staff member must review the PAR-Q and health history forms prior to scheduling your fitness assessment. Your trainer will also discuss your forms with you at your fitness assessment. 
  • All participants over 45 years old for males and 55 years old for females will need a medical release from a doctor.

Upon completion, you will be contacted by a GIT FIT staff member within three business days. We do our best to match you with a trainer as soon as possible. Most clients are matched with a trainer within a week or two after all paperwork has been submitted. Please list a minimum of three days of the week with times you're available. (Example: Mon/Wed, 3 p.m. - 6 p.m. or Tues/Thurs, 1 p.m. - 5 p.m.)
